Compassionate in-home care

Care that meets the family where it is

Memory changes rarely arrive on a schedule, and they rarely affect only the person who has them. Appointments get harder to keep. Behavior shifts in ways that are difficult to explain over the phone. The people doing the caregiving carry more and more of it alone.

Our dementia program brings the evaluation, the medical and psychiatric care, and the caregiver support into the home — the place where the day-to-day realities of dementia actually show up. Seeing the household, the routine, and the medicine cabinet in person tells us things an office visit cannot.

Because our team covers both geriatric primary care and psychiatric care, the medical side and the behavioral side of dementia are handled together rather than split between practices that never speak to each other.

Ideal for patients with:

  • New memory concerns or worsening confusion
  • Alzheimer's, vascular, Lewy body, or frontotemporal dementia
  • Behavior changes such as agitation, paranoia, wandering, or sleep disruption
  • Difficulty attending office visits, or a caregiver approaching burnout

How the care works

Three steps, start to ongoing care

The first visit establishes the full picture. Everything after it is built on that.

  1. Memory Evaluation

    • History and cognitive interview
    • Medication and polypharmacy review
    • Cognitive screening as clinically indicated
    • Mood and behavior review
    • Functional status
    • Home-safety and fall-risk evaluation
  2. Medical, Psychiatric & Behavioral Care

    • Dementia medication management
    • Care for sleep, mood, hallucinations, anxiety, and agitation
    • Review of reversible causes
    • Follow-ups by telehealth or in the home
  3. Caregiver, Nursing & Specialty Services

    • Education on what to expect at each stage of dementia
    • Coaching on identifying and defusing behavior triggers
    • Support around paranoia, anxiety, agitation, and grief
    • Respite and transition planning
    • Nursing visits
    • Specialty services available as add-ons

Why families choose us

One team for the whole picture

  • Medical, mental health, and behavioral care together

One team manages the physical health, the psychiatric care, and the behavior — so nothing falls into the gap between specialties.

  • Geriatric and psychiatric dementia expertise

Care is led by a provider board-certified in both psychiatric mental health and adult-gerontology.

  • Home-safety and caregiver-centered planning

Plans account for the home itself and for the people providing the care, not only the patient.

  • Coordination with your other providers

We work alongside primary care physicians, neurologists, facilities, hospice, and home health teams.

  • Culturally sensitive, spiritually inclusive

Care is delivered in English, Spanish, and Thai, and shaped around what matters to your family.

  • Grounded in established practice

Our approach draws on established geriatric care frameworks, adapted to the realities of each home.

What to expect

Visits with room to actually talk

Dementia care does not fit into a fifteen-minute slot. Our visits are scheduled with enough time to examine, to ask, and to listen to the family in the room.

Initial evaluation
75–90 minutes
Ongoing visits
30–60 minutes
Nursing and aide visits
Weekly or in scheduled blocks

After the initial evaluation, your family receives a practical care plan — what we are treating, what to watch for, and what to do next.
